Therefore, I am re-posting Dr. Eppley's original operative report which lists an anesthesiologist who was not present for my surgery along with a letter from this doctor confirming this fact; the consent form in which I made changes regarding administration of general anesthesia; and the anesthesia record illegibly signed by an individual whose identity and qualifications were never revealed to me.
